If you have arrived to access the Site Agreement, the Site Agreement is maintained on the HAT website. Please be sure that you have already created a login to our site. If not, you may do so by filling out the Website Application form that is here.

Please Note:
All HAT specific files such as protocol, consent forms and enrollment forms are now located at the HAT website. You must have a login name and password to access those files. To be issued a login and password, fill out and mail or fax the form listed here. |

Instructions: To download the files to your computer,
right-click on the name of the file (MAC users, hold down the Control
key and then click on the link). A menu will appear giving you an option to
"Save Target As", "Save Target to Disk", or "Download Link to Disk". You can then
choose where you would like the file saved and the browser will proceed to copy
the file from our server to your computer. When you click on file above
your computer may open the file within the browser or it may ask you if you want
to open the file or save it. If this happens, choose to save the file somewhere on
your computer.
